ONLINE APPLICATION FOR CERTIFICATES ISSUED BY PHARMACY BOARD MALAYSIA (PBM)

Effective from 2nd May 2018 applicants are advised to use PRiSMA at http://prisma.pharmacy.gov.my to apply for certificates issued by PBM

Please be informed that currently the online payment is not available for the applications that involves payment due to some inevitable issues. However alternatively payment can be done using postal order/money order/bank draft. We would like to apologise for the inconvenience cause. Thank you.

Announcement on Importation of Polymyxin including polymyxin B and polymyxin E (Colistin)

Please be informed that effective from 1st March 2018, Import Permit (e-Permit) under Customs (Prohibition of Imports) Order is required for every importation for Polymyxin including polymyxin B and polymyxin E (Colistin).

Aktiviti Penyelenggaraan Sistem Program Perkhidmatan Farmasi

Adalah dimaklumkan, bermula tarikh 29 September 2017 jam 5.00 petang sehingga 2 Oktober 2017 jam 12 tengah hari akan berlaku gangguan terdapat semua sistem di bawah Program Perkhidmatan Farmasi. Ini bagi memberi laluan kepada kerja-kerja penyelenggaraan elektrik untuk peningkatan mutu perkhidmatan Kerajaan sedia ada.

Credentialing Application

Please be informed that the Credentialing Committee Meeting No. 2/2016 Pharmacy Practice will be held in December 2016. Those who are interested to apply for credentialing, please submit your application to Pharmaceutical Services Divisions before or on 25 November 2016.

Any application received after 25 November 2016 will be brought to the next meeting in 2017.

Announcement on Importation & Exportation of Alpha-Phenylacetoacetonitrile (APAAN), Importation of Phosphorus and Importation of Nicotine

Renewal of Annual Certificate for pharmacist (AC) for the year of 2017

B)  Non-Practising Pharmacist (Not working / Practising in overseas)

  • CPD point is not required

Please be informed that effective from 1st November 2015, e-PERMIT under Customs Order (Prohibition of Import) and Customs Order (Prohibition of Export) is required for every importation and exportation for mercury and mercury compounds covered under Minamata Convention.
